Licensed Practical Nurse (LPN) – Home HealthFull-Time | Part-Time | PRNExcellent Pay + Paid Mileage + Flexible Scheduling

Freedom at Home is a Veteran-Owned Home Health Agency expanding into Northwest Ohio, and we're looking for compassionate, motivated Licensed Practical Nurses (LPNs) to join our growing team.

Responsibilities

  • Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse.

  • Follow and carry out individualized Plans of Care established by the supervising RN.

  • Perform skilled nursing visits and complete accurate, timely clinical documentation.

  • Monitor patient status and promptly communicate changes to the supervising RN and healthcare team.

  • Educate patients and caregivers on medications, treatments, disease management, and self-care techniques.

  • Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes.

  • Participate in case conferences, staff meetings, and quality improvement activities.

  • Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.

Qualifications

  • Graduate of an accredited Licensed Practical Nursing (LPN) program.

  • Current, unrestricted Ohio Licensed Practical Nurse license.

  • Current CPR certification.

  • Minimum of one year of LPN experience required.

  • Home Health experience is preferred but not required.

  • Valid driver's license, reliable transportation, and auto insurance.
