A good physical therapist is one who can develop an achievable plan of care and help a patient through that plan. A great physical therapist is one who develops a realistic plan with measurable goals tailored to each patient's situation, with consideration to the physician, family, and other healthcare providers. A great physical therapist doesn't just go through the motions, instead listening carefully to a patient's concerns, monitoring progress, and adjusting plans as needed. At Franciscan Health, we are looking for a great physical therapist who demonstrates these important qualities.

WHAT YOU CAN EXPECT

  • Complete appropriate goal re-assessment and discharge planning.
  • Perform comprehensive physical therapy evaluations, treatment, and related follow-up services, including discharge planning.
  • Document patient assessments and pertinent information in a timely manner.
  • Provide services and treatments requiring physical therapy skills, according to the written plan of care and the signed plan of treatment.
  • Supervise Physical Therapist Assistants and Home Health Aides.
  • Train and educate patients and family members or others support, and document understanding of treatment plan and goals.

QUALIFICATIONS
  • Bachelor's Degree- Physical Therapy - Required
  • 1 Year Physical Medicine/Rehabilitation Experience- Required
  • 1 Year Home Health Experience - Preferred
  • Physical Therapist (PT) State Licensing Board - Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
  • Driver's License - Required
  • Auto Insurance - Required
