Role Description The Certified Occupational Therapy Assistant (COTA) at Foursquare Healthcare is a full-time, on-site role based in Farmersville, TX. The COTA will assist in implementing occupational therapy treatment plans under the supervision of a licensed Occupational Therapist, focusing on improving residents’ functional abilities and independence. Daily responsibilities include carrying out therapeutic activities, monitoring patient progress, documenting treatments, and communicating changes in condition to the therapy and nursing teams. The role also involves supporting rehabilitation programs, educating residents and caregivers on techniques to enhance daily living skills, and participating in interdisciplinary care planning within the facility.
Qualifications

• Demonstrated skills in Occupational Therapy and Rehabilitation, with the ability to implement treatment plans and therapeutic activities.
• Experience in Patient Care and Home Care, including assisting with activities of daily living and providing education on safe, functional routines.
• Ability to contribute to Program Development, such as group activities or specialized therapy programs tailored to resident needs.
• Current certification as a Certified Occupational Therapy Assistant (COTA) and licensure to practice in the state of Texas.
• Strong communication, documentation, and collaboration skills for working within an interdisciplinary healthcare team.
• Ability to safely assist residents with mobility, transfers, and adaptive equipment, following facility and regulatory guidelines.
• Previous experience in skilled nursing, rehabilitation, or long-term care settings is preferred.
