Key Responsibilities

• Utilize your education and training to conduct comprehensive psychosocial assessments, formulating independent diagnoses that inform treatment strategies.
• Develop collaborative, patient-centered treatment plans that reflect individual needs, preferences, and goals.
• Implement evidence-based interventions, such as solution-focused treatment and motivational interviewing, ensuring the highest standards of care through measurable outcomes.
• Actively coordinate with multi-disciplinary teams, including other clinicians and primary care specialists, to provide a holistic approach to patient care.
• Monitor patient progress by systematically reviewing treatment responses, adjusting plans as necessary based on direct feedback and clinical observations, both virtually and in person.
• Support the management of psychotropic medications prescribed by collaborating medical providers, focusing on treatment adherence and addressing any potential side effects encountered by patients.

Patient Population / Client Focus

As a Licensed Clinical Social Worker at Fort Behavioral Center, you will engage primarily with patients in hospice care—individuals facing end-of-life challenges and their families. Your work will include addressing complex emotional and psychological needs, advocating for compassionate care, and providing the necessary support to foster a sense of peace and dignity during this critical stage of life.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• Current, valid, and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) in California.
• A minimum of one year of experience as an independently licensed therapist, preferably in clinical or hospice settings.

Preferred Experience

While the minimum experience requirement is one year, candidates with additional years of clinical experience, particularly in hospice or palliative care, will be at an advantage. Familiarity with electronic health records (EHR) systems and knowledge of community resources is beneficial.

Compensation and Benefits

The salary range for this position is competitive, set between USD 80,000 to 100,000 annually, commensurate with experience. In addition, eligible candidates will receive a sign-on bonus upon joining our team. We offer a comprehensive benefits package that includes:
