Position Overview
A leading home healthcare organization is seeking a Registered Nurse to provide comprehensive nursing care to patients in their homes throughout Monmouth County. This role offers the opportunity to build meaningful one-on-one relationships with patients while delivering high-quality clinical care in a home-based setting.
The Registered Nurse will be responsible for assessing patient needs, developing and implementing care plans, coordinating services with interdisciplinary team members, and ensuring compliance with regulatory and physician-directed care requirements.
Key Responsibilities

  • Conduct patient assessments and develop individualized care plans.
  • Deliver skilled nursing care in accordance with physician orders, agency policies, and professional standards.
  • Monitor patient progress and adjust care plans as needed to support optimal outcomes.
  • Complete medication reconciliation and collaborate with physicians regarding treatment plans.
  • Ensure accurate, timely, and compliant clinical documentation.
  • Educate patients and caregivers using appropriate teaching methods and document educational interventions.
  • Coordinate care with physicians, case managers, therapists, home health aides, and other healthcare professionals.
  • Monitor authorization requirements and maintain compliance with payer guidelines.
  • Report patient concerns, changes in condition, and care issues promptly to appropriate personnel.
  • Supervise home health aides and complete required supervisory visits within regulatory guidelines.
  • Participate in patient care transitions and communicate care plans effectively among team members.
  • Maintain productivity expectations and complete assignments independently and accurately.
  • Stay current with nursing standards, clinical guidelines, and best practices.
  • Perform additional duties as assigned.
Required Qualifications
  • Associate Degree in Nursing (ADN) or Diploma in Nursing.
  • Minimum of six (6) months of Registered Nurse experience OR at least one (1) year of Licensed Practical Nurse experience.
  • Experience in home health or hospice care preferred.
  • Basic computer proficiency and documentation skills.
  • Active New Jersey Registered Nurse (RN) license.
  • Current AHA Basic Life Support (BLS) for Healthcare Providers certification.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN).
  • Prior home health nursing experience.
  • Strong experience in care coordination and patient education.
