Social Work Manager
Location: Fall River Health Services - Hot Springs, SD
Schedule: Monday - Friday 40 hours a week.
Fall River Health Services is seeking an experienced Social Work Manager to lead our Social Worker department. The Social Work Manager leads our patient advocacy, counseling, and discharge planning services. This position will split time between hospital and SSLC operations as needed to facilitate the needs of clients and patients. In a rural healthcare setting, this role is vital for connecting patients with limited regional resources. The Social Work Manager is a hands-on, working manager and will oversee daily social work operations across the hospital and clinic, manage a small team, and provide direct support to patients and families navigating complex healthcare choices.
What You'll Do
Department Leadership & Continuity
Staffing Coverage: Provide direct social services coverage when department staff are out of the office. Manage the daily operations of the social work department and supervise social work staff.
Succession Planning: Crosstrain with current social workers and develop a formal succession plan to ensure seamless departmental operations.
Compliance & Ethics Oversight: Provide continuity and oversight for Centers for Medicare & Medicaid Services (CMS) compliance, care conferences, and grievance processes. Ensure all social work activities meet HIPAA privacy rules, state laws, and healthcare quality standards.
Clinical & Continuum of Care Operations
Acute & Swing Bed Care: Oversee hospital Medicare and swing bed referrals, discharge planning, and care coordination.
Long-Term Care (LTC) Support: Support long-term care Medicaid processes, admissions, discharge planning, and strict regulatory requirements. Coordinate safe, timely discharges from the LTC to home or other care setting.
Cross-Departmental Assistance: Assist the Emergency Department and rural health clinic with social work and crisis needs as necessary.
Crisis Intervention: Provide immediate emotional support, counseling, and advocacy for patients and families during medical emergencies.
Patient Advocacy: Serve as a liaison between patients, medical staff, and insurance providers to protect patient rights.
Strategic & Revenue Growth
• SSLC Performance: Improve timely referral reviews and strategic discharge planning at Seven Sisters Living Center (SSLC).
Census Optimization: Drive operational efficiencies to allow for quicker Medicare admissions.
Rural Resource Coordination: Connect patients with regional housing, transportation, financial aid, and mental health resources.
Qualifications
  • Education: Bachelor of Social Work (BSW) from an accredited program required. Master of Social Work (MSW) preferred.
  • Licensure: Active Social Work License in the State of South Dakota required. LCSW or CSW preferred.
  • Experience: Must have verifiable experience in Social Work. Prior management and healthcare experience is preferred.
  • Methodologies: Strong knowledge of CMS guidelines, Swing Bed programs, Crisis Intervention Models, Trauma-Informed Care, and Case Management strategies.
  • Communication: Exceptional empathy, verbal communication, and conflict-resolution skills.
  • Problem Solving: Strong ability to handle complex medical, social, and legal issues including Crisis De-escalation, Resource Allocation, Software Navigation, Conflict Resolution, Ethical Decision-making, and Root Cause Analysis.
  • Leadership: Proven track record of managing teams and driving workplace safety.

Requirements
Education/Experience/Licensure:
