Licensed Practical Nurse (LPN) - Fall River Health Services

Wage starts at $27.00 and goes up from there based on experience.

Full-time, part-time, and PRN hours available

Fall River Health Services is hiring a Licensed Practical Nurse (LPN) to join our multidisciplinary care team in Host Springs SD! This position offers the chance to provide compassionate, high-quality care in a supportive and team-oriented environment.


About the Role

As an LPN, you'll provide general nursing care to residents and play a key role in ensuring safe, effective, and person-centered care. You'll work closely with the nursing team, physicians, and families to meet the unique needs of each resident.


Responsibilities
  • Provide nursing care following physician orders and LPN scope of practice.

  • Supervise and guide Nursing Assistants during assigned shifts.

  • Coordinate and oversee resident care plans in collaboration with the Director of Nursing or Charge Nurse.

  • Administer and chart medications and treatments accurately and safely.

  • Maintain complete and timely documentation to support continuity of care.

  • Communicate effectively with residents, families, and healthcare providers.

  • Assist with resident assessments, care planning, and evaluation.

  • Promote infection control and safety standards.

  • Participate in the Quality Management Program to ensure compliance with care standards.


Qualifications
  • Current Licensed Practical Nurse (LPN) license in South Dakota (or eligibility for reciprocity).

  • Strong communication, leadership, and clinical skills.

  • Experience in long-term care or skilled nursing preferred.

  • Commitment to resident rights, dignity, and quality care.
