Job description

SUMMARY: Support clients by drawing on lived experience to provide services as defined by CA State Department of Mental Health, including but not limited to: case management for client including planning, services, monitoring, coordinating with other agencies and other activities as required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential job functions.

ESSENTIAL DUTIES AND RESPONSIBILITIES Must be able to perform each essential job function satisfactorily.

Direct Service and Case Management

  1. In a culturally competent manner will provide services to all clients without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, and handicap or payment status.
  2. Provide clients with counseling and training; assistance with daily living skills and managing psychiatric symptoms.
  3. Assist in identifying and accessing medical, educational, prevocational, rehabilitative, social and other community services, specific to clients’ needs, goals, and service plans.
  4. Initiate preventative measures and create intervention methods for clients or conditions that pose a threat of danger or crisis.
  5. In conjunction with client, family and providers prepare Annual Assessments
  6. Inform individuals of their personal rights, responsibilities and Company’s grievance procedures. Ensure services are compatible with clients’ freedom of choice.
  7. Transport clients to appointments, outings, etc. Maintain vehicle inspection logs.
  8. Provide information to clients, families, other agencies and the community regarding health care issues including prevention, early detection and remedial care.

Teamwork and Collaboration

  1. Maintain cooperative and professional working relationships and communications.
  2. Give input to management on improving the quality of programs and assists in monitoring program effectiveness and conduct client satisfaction surveys.

Reporting and Documentation

  1. Mandated to report any “suspected abuse of dependent adults and elders” directly to law enforcement, Adult Protective Services, Ombudsman, or Community Care Licensing.
  2. Monitor clients’ progress and participation toward service plan goals, collect and report outcome data. Document coordination and complete all documentation (E.g. annual assessments, service plans and progress notes) within required timelines.
  3. Keep clear, accurate, and professional records in client’s files, staff logs, and other required communications.
  4. Meet SCCMH and FSI guidelines for documentation and attend QIC as needed. 

QUALIFICATION REQUIREMENTS:

  • The ideal candidate will be a consumer of behavior health services but those with lived experience are welcome to apply.
  • Must be at least 18 years of age.
  • If required to drive, must be 21 and accepted by the Company’s vehicle insurance.
  • Pass criminal background clearance prior to starting with HS Team.
  •  Maintain a valid First-Aid certification.
  • Complete all annual training requirements as mandated by Front St. policies.
