About the Role:

The Home Health Community Liaison serves as a vital connection between home health care providers and the community, ensuring that patients receive seamless access to necessary health services. This role focuses on building and maintaining strong relationships with healthcare professionals, community organizations, and potential referral sources to promote home health programs. The liaison will assess community needs, educate stakeholders about available services, and facilitate communication to optimize patient care coordination. By effectively advocating for patients and providers, the liaison helps to improve health outcomes and supports the growth of home health services within the community. Ultimately, this position plays a key role in enhancing patient access to quality home-based care and fostering collaborative partnerships.

Minimum Qualifications:

  • High school diploma or equivalent; Associate’s or Bachelor’s degree in healthcare, social work, or related field preferred.
  • Minimum of 2 years experience in healthcare, community outreach, or patient advocacy roles.
  • Strong knowledge of home health care services, healthcare systems, and community resources.
  • Excellent communication and interpersonal skills to effectively engage diverse stakeholders.
  • Valid driver’s license and reliable transportation for community visits.

Preferred Qualifications:

  • Bachelor’s degree in healthcare administration, social work, nursing, or related discipline.
  • Experience working in home health care or case management.
  • Familiarity with electronic health records (EHR) and referral tracking software.
  • Bilingual abilities to serve diverse populations.
  • Certification in patient advocacy or community health outreach.

Responsibilities:

  • Develop and maintain relationships with hospitals, physicians, social workers, and community organizations to generate patient referrals for home health services.

  • Collaborate with interdisciplinary teams to coordinate patient care plans and ensure smooth transitions from hospital to home health services.
