RN MDS Coordinator - Full-Time
Delaware, OH
Join a skilled nursing team that values clinical excellence, teamwork, and resident-centered care. We're seeking an experienced MDS Coordinator to oversee the RAI process, support reimbursement accuracy, and help drive quality outcomes for our residents.
What You'll Do
  • Coordinate and complete MDS assessments
  • Manage the RAI process and care planning Support Medicare, Medicaid, and Managed Care reimbursement
  • Collaborate with nursing, therapy, dietary, and social services
  • Ensure compliance with state and federal regulations
  • Monitor PDPM and quality measures
Qualifications
  • Current Ohio RN License
  • MDS experience preferred
  • Knowledge of PDPM and reimbursement systems
  • Strong organizational and communication skills
  • Skilled nursing experience preferred
