Job description

JOB PURPOSE: The Case Aide supports the Case Management team with administrative and client support activities to enhance service coordination and client engagement.ESSENTIAL JOB RESPONSIBILITIES:
  • Assists with distribution of caseload to ensure all cases are accounted for in the allotted time frame.
  • Assist clients with appointments, forms, and documentation.
  • Support transportation coordination and referrals.
  • Assist with data entry and HMIS documentation.
  • Participate in scheduled meetings and trainings.
  • Conduct client follow-ups and appointment reminders.
  • Support resource distribution and client engagement activities.
  • Maintain organized client files and records.
  • Provide general administrative support to the case management team.
  • Other duties as assigned

   ESSENTIAL QUALIFICATIONS:

EDUCATION: High School or GED diploma.

EXPERIENCE: Minimum of 1 year of related experience in social services, customer services, or administrative support preferred. Strong research and technical writing skills required.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion. Ability to work flexible schedules including weekends, holidays, and overnight shifts.

LICENSES: Driver’s License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
