Position Summary

The Licensed Practical Nurse (LPN) provides compassionate, high-quality nursing care that promotes resident health, comfort, dignity, and independence. Working under the direction of the Registered Nurse and in collaboration with the interdisciplinary care team, the LPN delivers direct resident care, administers medications and treatments, monitors resident conditions, and helps ensure each resident receives personalized, evidence-based care.

Key Responsibilities

• Administer medications, treatments, and nursing interventions in accordance with physician orders and facility policies.
• Monitor resident conditions, assess changes in health status, and promptly communicate concerns to the appropriate healthcare providers.
• Record vital signs and maintain accurate, timely, and compliant clinical documentation.
• Assist residents with activities of daily living while promoting dignity, comfort, and independence.
• Collaborate with physicians, Registered Nurses, Certified Nursing Assistants, therapists, and the interdisciplinary care team to develop and implement individualized care plans.
• Provide education, emotional support, and compassionate communication to residents and their families.
• Ensure compliance with infection prevention, safety, and regulatory standards.
• Respond appropriately to resident emergencies and changes in condition.
• Participate in quality improvement initiatives, staff meetings, and ongoing professional development.
• Foster a positive, respectful, and resident-centered environment for residents, families, and fellow team members.

Requirements:Required Qualifications

• Current Licensed Practical Nurse (LPN) license in the State of West Virginia.
• Current CPR certification (or ability to obtain within required timeframe).
• Strong knowledge of nursing principles, medication administration, and resident assessment.
• Excellent communication, interpersonal, and organizational skills.
• Ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
• Commitment to compassionate, resident-centered care.
• Ability to work independently while collaborating effectively as part of an interdisciplinary healthcare team.
• Ability to meet the physical demands of the position, including lifting up to 50 pounds, prolonged standing and walking, bending, pushing, pulling, and assisting residents with mobility and daily care needs.

Preferred Experience

• Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute care.
• Experience caring for geriatric residents with complex medical needs.
• Knowledge of electronic medical records (EMR) and nursing documentation systems.
• Experience with medication administration, wound care, restorative nursing, or dementia care.
• Strong clinical judgment, time management, and critical thinking skills.
• Passion for providing exceptional care while supporting residents' highest quality of life.
