Key Responsibilities

• Conduct detailed evaluations and assessments of patients requiring physical therapy as part of a home care or outpatient rehabilitation setting.
• Develop and implement individualized treatment plans that utilize evidence-based methodologies, including exercise therapy, manual techniques, and modalities to address pain management and mobility improvement.
• Monitor and document treatment effectiveness, adjusting strategies as necessary to meet the evolving needs and therapeutic goals of each patient.

• Facilitate communication among healthcare team members regarding patient progress and modifications in treatment plans.

Qualifications

• Must possess a Doctorate in Physical Therapy (DPT) from an accredited educational institution.
