The Coordinator works with Direct Support Professionals (DSPs) to ensure delivery of quality community-based services.ESSENTIAL JOBFUNCTIONS:
Supervision of Employees responsible for direct care.
Complete andmaintainattendance point tracker.
Check employee's timeinworkday for accuracy.Submit when due to payroll.
Obtain approval to provide discipline, draft performancedocumentationand deliver to employees.
Create employee requisitions.Interview, hire, and train employees.
Workwith Directorsto ensureaccuratebilling data forthearea; including by not limited to:
Evaluating staffing in locations toensureall needs are met using funding levels provided by the State.
Ensuring daily attendance meets serviceauthorizationcriteria.
Evaluate that all EVV checks in and outs are present.
Monitor monthly attendance for all services provided to ensure billing does not result in repayment to the State.
Assistwith building and fostering a culture that:
Empowers direct reports tothink outside the boxwhile meeting new challenges.
Tempershigh expectationsfor job performance with kindness.
Holds people accountable with respect, ensuring HR documentation is completed.

Ensures that all employees receive transparent communication, so they are informed and understand job expectations. This should be done byfacilitatingmonthly staff meetings and using frequent communication emails.
Assure that people's needs are met, while taking a team approach todoso in an efficient and cost-effective manner.Some examples include but are not limitedto:medical appointments, transportation, snow days,etc.
Works as part of a team tofoster creativity throughout areas for growth, business development, and person-centered planning.
Rotates on-call responsibilities with other team members
Covers vacant shifts when needed.
Assure that Agency policies and procedures are adhered to including but not limitedtothe following:abuse and neglect, financial documentation, and human resources.
Be visible and have open communication with employees, those receiving services, families, and State employees. This visibility extends throughout all Area physical locations and all services provided.
Check and respond to emails and voicemails within 24 hours of receiving them.
Check T-Logs.
Contact Guardians and families at leastoncea month unless they specifydifferent frequencies.
Communicate by each person's preferred method of communication, such asemail, phone, mail, etc.
As requested by the Area Director, visit each environment assigned to yourteamvarying shifts to interact with as many employees and peoplesupported as possible.On-sitevisits must includeweekends, evenings,and morningshiftsto the extent possible.
Complete all required documentation thoroughly and accurately within established timelines.
Check and respond to SCOMM's.
Complete GER for all reportable incidents.
Contact Service Coordination for all required tasks such as GER, investigations, etc.
Initiatemaintenance and vehicle requests.
Check emails and workday tasks daily.
Check andsubmitemployee mileage, strategizing efficient use of area vehicles.
Establish andmaintainconsistent lines of communication with all stakeholders.
Promote a proactive approach when facing change.
Be directly involved with and promote inter-area collaboration of the planning, implementation and maintenance of projects that meet and exceed HCBS requirements.
Activelyparticipatein meeting goalsset forth inthe Agency's Strategic plan.
Willing to be flexible with schedule and put in the necessary time to ensure superior service delivery.
Able to work independently and in a self-directed manner.
Ensure that all compliancetasksincludingdocumentationare completed accurately and within the established timelines.
Write habilitation programs following SMART goal criteria.
ensure documentation is beingcompleted per frequencydecided onby the ISP team.Completes monthly goal reviews.
Check HCBS log and CCIR tracking (day service) for completion.
Review Service Needs / Staffobjectives.
Completes reviews of restrictions
Facilitate completion of assessments.

Assuresthe following are completed: Health care report, health tracking review,andMAR reports.
Reviews Health-related service needs and ensures medical appointment follow-up.
Isresponsible forassuring fiscal-related needs areaddressed.
Assure private pay is billed accurately when applicable.
Complete the SNAP audit and verify the balance.
Audits grocery credit cards.
Check True link cards todetermineif any transactions occurred.
Check all debit card transactions and make sure they are documented in Therap.
Make sure all receipts are accounted for and in Therap.
Establish andmaintainconsistent lines of communication with all stakeholders.
Review and acknowledge T-logs.
Check and respond to emails, SCOMMs, and voicemails within 24 hours.
Attend staff meetings for the team.
OTHER JOB DUTIES:
Other duties as assigned.
EDUCATION, TRAINING AND EXPERIENCE:
Must be at least 19yearsof age.
Musthave a high school diploma or GED.
ESSENTIAL KNOWLEDGE,SKILLSAND ABILITIES:
1 year of management experience in community-based services preferred.
Mustpossessand maintain a current, valid driver's license, pass a driving record check, and may, depending on Agency needs,be requiredto have available an automobile with required state insurance coverage for purposes of transporting people in service.
Ability toutilizeMicrosoft Office Suite and all Agency required systems.
PHYSICAL DEMANDS:
In this job capacity you will be fully submerged intosupported persons' home and community, including but not limited to stores, movie theaters, medical facilities, places of worship, hobbies and interests, person centered celebrations, outdoors (gardens, parks, etc.) and restaurants.
A pre-employment physical and backscreen will be completed to qualify you for the essential functions of the job.
Must be able to lift at least 70 pounds, toassistsupported individuals in everyday tasks that require mobility they may be limited to (i.e.assistingin/out of wheelchair, in/out of vehicles, Hoyer lift, raised toilet seats, etc.).
Must be able tobend, stoop, kneel, squat, walk, sit, stand, climb, push, carry, and grasp.
