JOB SUMMARY

The Physician is a member of the interdisciplinary health team of FHCSO and works in collaboration with the Medical Director/Clinical Director, and their physician team, as well as supervising advance practice providers who provide primary care. The Physician provides advance practice providers with consultation as needed, provides primary care for clients in collaboration with other center physicians. The Physician also teaches health professions, students and residents.

RESPONSIBILITIES
  • Participates in the provision of first contact care and coordination of continuity of care to clients of all ages.
  • Assumes responsibility for the care of a group of FHCSO clients.
  • Completes the documentation on patients on a daily basis.
  • Participates in the coordination of community agencies and/or resources in providing continuity of care.
  • Participates in the continuing education of the medical staff.
  • Conducts and/or participates in-group educational experience for clients and families.
  • Participates in individual client or family education or counseling activities in collaboration with other members of the health center interdisciplinary team.
  • Participates appropriately in health center staff activities.
  • Provides advance practice providers with consultation and supervision. Reviews and countersigns medical records with them.
  • Will be responsible for conducting meetings for their team in accordance with PCMH standards.
  • Participates in any other designated activities and projects for PCMH as assigned by the Chief Medical Officer.
  • Assists with development and implementation of medical care policies and procedures to ensure compliance with existing standards, practices, and funding sources regulations.
  • Assists with overall coordination and administration of center health care services and activities including clinical indicators, health care plan and quality assurance.
  • Responsible for a chief complaint, history of present illness, patient profile, past medical history, family history and review of systems. This includes the social, economic and emotional data.
  • Performs a physical examination, using the skills of auscultation, percussion, palpation and inspection.
  • Establishes a problem list and plan of therapy which includes teaching and counseling, ordering and performing laboratory and diagnostic tests for baseline data, diet, pharmacotherapy, activity using problem oriented medical records system.
  • Determines final disposition of client.
  • Makes appropriate referrals to other resources when needed.
  • Provides follow-up care for monitoring health and disease state, evaluating progress of each problem including patient education.
  • Participates in clinical audit.
  • Provides after-hours coverage as outlined by policy of center.
  • Provides for health center clients in collaboration with other center physicians
  • Performs other related duties incidental to the work described herein.
  • Aids in making patient services more accessible, effective, and efficient, and strives to minimize unneeded costs.
  • Maintains up to date required state licensure and certifications.
  • Follows the mission and vision statement.
  • All other duties as assigned.
QUALIFICATIONS
  • Graduate of formal accredited medical or osteopathic school.
  • Must be licensed by Oklahoma State Board of Licensure of Supervision.
  • Has completed Residency in Primary Care/Internal Medicine/Surgery.
  • Interest and/or experience in collaboration with interdisciplinary health teams.
  • Possesses medical skills at a high level of competency.
  • Maintains current continuing medical education and requirements for privileging.
