Job description

Critical Connection, inc is seeking a travel Skilled Nursing Facility (SNF) Occupational Therapist for a travel job in Elk Grove, California.

About the Opportunity

We are seeking an Occupational Therapist (OT) passionate about helping patients regain independence following hospitalization or illness in a skilled nursing setting.

This role focuses on functional recovery, ADLs, and safe discharge planning. New grads encouraged to apply.

What You’ll Do

  • Evaluate and treat patients with functional, cognitive, and physical impairments
  • Address ADLs, IADLs, upper-extremity rehab, and adaptive strategies
  • Provide 1:1 patient-centered care
  • Collaborate with PT, SLP, nursing, and case management
  • Complete accurate documentation and discharge planning

Qualifications

  • Master’s or Doctorate Degree in Occupational Therapy
  • Active (or pending) state OT license
  • SNF experience preferred, not required
