Work Environment

Occasional evening or weekend hours may be required to meet client needs and program activities.

Key Responsibilities

• Conduct thorough, comprehensive assessments to identify clients’ social, emotional, and environmental challenges, ensuring a tailored approach to each individual’s needs.
• Develop personalized service plans that include specific goals, interventions, and mechanisms for progress monitoring, thereby fostering client empowerment.
• Deliver individual and family counseling, employing evidence-based practices and techniques that facilitate healing and recovery.
• Provide crisis intervention services, conducting risk assessments, and formulating safety plans to protect clients in vulnerable situations.
• Engage in collaborative dialogue with clients to connect them to vital community resources and services that address their immediate challenges.
• Participate actively in community outreach initiatives aimed at raising awareness about agency services and bridging gaps in social service availability.
• Attend and contribute to case conferences, team meetings, and ongoing staff training to ensure continuous improvement in service delivery and team dynamics.

Patient Population / Client Focus

As an LCSW at Compassionate Connections Clinic, you will primarily serve diverse populations facing significant social, economic, and emotional hurdles. Your work will focus on assisting individuals and families impacted by homelessness, domestic violence, mental health issues, and substance use disorders. By developing trust and rapport, you will be instrumental in helping clients navigate their challenges and achieve their personal goals, thereby improving overall community health outcomes.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required to ensure a high standard of professional practice.
• An active state license as a Licensed Clinical Social Worker (LCSW) is mandated; this verifies your qualifications to practice independently and provide clinical services.
• Strong communication skills are vital, with an emphasis on cultural competence and the ability to work with diverse populations.
