We are currently seeking a dedicated Nurse Practitioner (NP/APRN) to provide exceptional care within long-term care facilities serving Grand Island, Aurora, York, and potentially the Columbus area.

What You’ll Do

  • Perform comprehensive physical assessments and review patient histories
  • Diagnose and manage acute, chronic, and infectious conditions with an emphasis on geriatric chronic care
  • Order, interpret, and manage laboratory and diagnostic testing
  • Provide wound care management once credentialed
  • Prescribe and manage medications and treatment plans
  • Educate patients and families regarding prevention, wellness, and ongoing care plans
  • Maintain timely and accurate EMR documentation using EClinical Works
  • Collaborate closely with interdisciplinary healthcare teams
  • Deliver proactive, compassionate, patient-centered care

Qualifications

  • Licensed APRN / Nurse Practitioner in Nebraska
  • Primary Care experience required
  • Wound Care credentialed OR willing to complete credentialing (company paid)
  • Strong clinical judgment and diagnostic skills
  • Knowledge of healthcare compliance and regulatory standards
  • Excellent communication and patient education abilities
  • Dependable, compassionate, and team-oriented mindset
