Work Environment

Key Responsibilities
  • Conduct comprehensive assessments of clients’ psychosocial needs, focusing on emotional, mental, and social factors that may affect their health and wellness.
  • Collaborate closely with clients and their families to create personalized care plans that are aligned with their specific goals, preferences, and needs.
  • Provide counseling and emotional support, helping clients navigate life's transitions, such as bereavement, health changes, or family dynamics.
  • Advocate for clients’ rights and access to various community resources, ensuring they have the support necessary to live fulfilling lives.
  • Work with interdisciplinary teams to implement and manage care plans, adjusting recommendations based on ongoing evaluations of client needs.
  • Monitor clients’ well-being through regular check-ins and assessments, addressing any changes in health conditions or living environments as necessary.
  • Educate clients and their families about available resources, enhancing their community engagement and opportunities for social interaction.
  • Maintain meticulous documentation of client assessments, treatment plans, and progress notes, ensuring compliance with ethical standards and regulatory requirements.
Patient Population / Client Focus

This role primarily focuses on assisting seniors in our community who may be experiencing challenges related to aging, including mental health issues, social isolation, or family dynamics. Your work will empower clients to maintain their independence and enhance their quality of life by connecting them to therapeutic resources and support systems.

Qualifications
  • A Master’s Degree in Social Work from an accredited institution is required.
  • Possession of current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum of two years of relevant experience in a social work capacity, particularly with seniors or in a community center environment.
  • In-depth knowledge of social work principles, strategies for aging-related issues, and familiarity with available community resources.
  • Proficient assessment, counseling, and problem-solving skills, demonstrating empathy and compassion.
  • Strong interpersonal and communication skills to effectively collaborate with a diverse range of stakeholders.
  • Proficiency in electronic health record (EHR) systems and other technology used in social service delivery.
Preferred Experience
  • Experience in geriatric social work or navigating complex healthcare systems.
  • Previous involvement with community service organizations or geriatric support groups.
  • Familiarity with evidence-based therapeutic practices for older adults.
