Cleveland Health Center is seeking an experienced, compassionate, and highly dependable Advanced Practice Registered Nurse / Nurse Practitioner (APRN/NP) to join our growing primary care team in the Wildwood/The Villages area.

We are a physician-led outpatient medical practice focused on providing high-quality, personalized care to our adult and older-adult patient population. The ideal candidate is clinically confident while remaining open to collaboration and feedback. They should be organized, accountable, personable with patients, and comfortable managing both chronic medical conditions and acute primary-care concerns.


What You'll Do


As a Nurse Practitioner with Cleveland Health Center, you will provide comprehensive outpatient primary care while working alongside our physician, other advanced practice providers, medical assistants, and administrative team.

Responsibilities include:

  • Evaluate, diagnose, and treat adult primary-care patients
  • Manage common acute and chronic medical conditions
  • Develop and implement evidence-based treatment plans
  • Perform routine follow-up visits and preventive care
  • Participate in Medicare Annual Wellness and preventive-care services
  • Order and interpret laboratory testing, imaging, and other diagnostic studies
  • Prescribe and manage medications within applicable scope of practice
  • Educate patients regarding their diagnoses, medications, preventive health, and treatment plans
  • Coordinate referrals and collaborate with specialists when appropriate
  • Review results and follow through on clinically important findings
  • Maintain accurate, timely, and thorough medical documentation
  • Participate in quality-measure and population-health initiatives
  • Communicate effectively with patients, families, physicians, clinical staff, and other members of the care team
  • Practice in accordance with applicable Florida laws, professional standards, and Cleveland Health Center policies


What We're Looking For


Required


  • Graduate of an accredited Nurse Practitioner program
  • Master's degree or higher in Nursing
  • Current and unrestricted Florida APRN license
  • Current national board certification appropriate for adult primary care, such as FNP or Adult-Gerontology Primary Care NP
  • Ability to obtain and maintain credentialing with Medicare and applicable commercial insurance plans
  • Current BLS certification
  • Strong clinical judgment and decision-making skills
  • Excellent communication and interpersonal skills
  • Professional, dependable, and accountable approach to patient care
  • Ability to work effectively as part of a collaborative healthcare team


Preferred


  • Two or more years of Nurse Practitioner experience in outpatient primary care, family medicine, or internal medicine
  • Nurse Practitioner experience in outpatient primary care, family medicine, or internal medicine
  • Experience caring for Medicare and older-adult patients
  • Experience managing multiple chronic medical conditions
  • Familiarity with value-based care and quality measures
  • Experience with eClinicalWorks
  • Active DEA registration, if applicable to the position


Exceptional candidates with less experience may also be considered if they demonstrate strong clinical knowledge, maturity, communication skills, and a commitment to primary care.
