Case Manager – RNCherokee Elder Care

Are you a compassionate, organized Registered Nurse who enjoys building relationships and helping older adults thrive? Cherokee Elder Care is seeking a dedicated Case Manager–RN to join our team and play a key role in supporting the health and well‑being of our PACE participants.

About the Role:

As a Case Manager–RN, you’ll work under the supervision of the Chief Medical Director and your Primary Care Provider to assess participant needs, coordinate care, and ensure high‑quality health services. You’ll serve as a vital member of our interdisciplinary team, helping guide care plans, support chronic disease management, and contribute to ongoing quality improvement efforts. This role is ideal for an RN who values teamwork, autonomy, and meaningful connections with participants and caregivers.

Key Responsibilities:
  • Complete initial nursing assessments, history reviews, and functional evaluations for new participants
  • Develop and update individualized nursing care plans
  • Collaborate closely with the interdisciplinary team to support holistic participant care
  • Monitor changes in participants’ health status and ensure the team stays informed
  • Administer medications and treatments, and provide education to participants and caregivers
  • Conduct triage by phone and in person
  • Document all nursing care per Cherokee Elder Care standards
  • Coordinate care with specialists and contracted services
  • Provide guidance and oversight to assigned C.N.A.s
  • Assist with staff orientation, required reporting, and quality improvement activities
  • Participate in care conferences, staff meetings, and in‑service trainings
  • Share on‑call responsibilities on a rotating basis (paid and all by phone, you do not leave the house for oncall responsibilities)
  • Maintain confidentiality and protect sensitive health information
  • Offer health counseling and support for chronic conditions and end‑of‑life care
  • Perform other duties as assigned by the Chief Medical Director
