Catholic Charities of Care Coordination Services is looking for a Harm Reduction Outreach Specialist who wants more than just a job โ€” youโ€™ll have the opportunity to empower individuals and families, strengthen community wellness, and see the direct impact of your work.

What Youโ€™ll Do

As a key member of our Project Safe Point team, you will:

  • Accept participant referrals from Albany County Correctional and Rehabilitation Facility

  • Make initial contact with eligible participants on site, as well as in the community with program participants

  • Deliver outreach and engagement services in accordance with the service plan

  • Work with Syringe Service Program staff to facilitate access to Harm Reduction supplies as needed

  • Conduct prevention related harm reduction/risk reduction interventions with participants

  • Register for and attend community trainings/workshops to assist with professional development

What Weโ€™re Looking For
  • HS Diploma or GED

  • Must have a personal cell phone, valid driverโ€™s license, proof of current car insurance and registration and access to a working, reliable vehicle.

  • Knowledge of Harm Reduction, Motivational Interviewing, HIV, and Hepatitis C.

  • Comfort and experience working with current or former substance users

  • Harm Reduction Outreach Specialist schedule: 37.5 hours a week, Mondayโ€“Friday 8:30AM - 4:30PM
