Job description

Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will provide high-quality patient care in a clinic setting. You will be responsible for developing individualized treatment plans, implementing therapeutic interventions, and collaborating with other healthcare professionals to ensure optimal patient outcomes. Your expertise in physical therapy techniques, including trigger point therapy and orthopedic care, will be essential in helping patients regain mobility and improve their quality of life. We are open Monday, Wednesday, and Friday from 9 am to 5 pm. We will potentially be adding Thursday to the work schedule.

Duties

  • Conduct comprehensive assessments of patients to determine their physical therapy needs.
  • Develop and implement effective care plans tailored to individual patient goals and conditions.
  • Provide hands-on physical therapy treatments, including exercises, manual therapy, and modalities as appropriate.
  • Educate patients and their families on treatment plans, home exercises, and lifestyle modifications to promote recovery.
  • Collaborate with interdisciplinary teams to coordinate care and discharge planning for patients transitioning from home community care or clinic settings.
  • Maintain accurate documentation of patient progress and treatment outcomes in compliance with regulatory standards.
  • Stay updated on the latest advancements in physical therapy practices and participate in professional development opportunities.

Experience

  • A degree in Physical Therapy from an accredited program is required.
  • Valid state licensure or eligibility for licensure as a Physical Therapist.
  • Experience in home health settings is preferred but not mandatory; training will be provided.
  • Familiarity with orthopedic conditions and discharge planning processes is advantageous.
  • Strong communication skills to effectively interact with patients, families, and healthcare teams.
  • Ability to develop comprehensive care plans that address the unique needs of each patient.
