We’re looking for a compassionate RN to provide skilled, patient-centered care in the comfort of patients’ homes.

We also use innovative AI-powered clinical documentation technology to help make charting more efficient, giving our nurses more work-life balance.

Responsibilities
  • Provide skilled nursing care in patients' homes
  • Complete patient assessments and monitor changes in condition
  • Administer medications and treatments
  • Perform wound care, IV therapy, catheter care, and other skilled procedures
  • Educate patients and families on medications, conditions, and care needs
  • Communicate with physicians and the care team
  • Coordinate and manage patient care
  • Complete timely and accurate clinical documentation
  • Follow all applicable clinical, HIPAA, and agency standards
Qualifications

PROFESSIONAL EXPERIENCE/QUALIFICATIONS:

  • Maintains the agency’s mission, philosophy and core values.
  • Adheres to dress code, appearance is neat and clean, wears identification while on duty.
  • Reports to work on time and/or as scheduled, notifies patients for delays in expected arrival times.
  • Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
  • Ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.
  • Always maintains patient confidentiality, including all HIPAA regulations.
  • Complies with agency “On-Call” requirements as specified by the Clinical Manager or Administrator.
Regulatory Requirements
  • Licensed or registered in the worked in state.
  • Must pass a criminal background check & MVR check.
  • Completed health statement acknowledging ability to perform the duties of the position.
  • Valid state driver’s license.
  • Proof of current automobile insurance.
  • Current CPR card
  • TB testing per agency policy
