Capstone Home Healthcare

Capstone Home Health is seeking a detail-oriented and organized Intake Coordinator to manage incoming referrals and coordinate the admission process for home health services. This role is essential in ensuring timely, accurate intake processing while delivering excellent communication with referral sources, patients, and internal staff.

Key Responsibilities

• Receive and process new patient referrals from hospitals, physicians, and community partners
• Verify patient information, insurance eligibility, and service needs
• Coordinate start of care (SOC) visits within required timeframes
• Communicate with referral sources regarding acceptance, delays, or additional needs
• Ensure all required documentation is obtained prior to admission
• Enter and maintain accurate patient information in EMR systems
• Collaborate with clinical staff to ensure appropriate service planning
• Track and follow up on pending referrals and incomplete documentation
• Maintain compliance with Medicare, Medicaid, and agency policies
• Provide exceptional customer service to patients and referral partners

Qualifications

• Experience in home health intake, medical office intake, or healthcare coordination preferred
• Knowledge of Medicare/Medicaid guidelines and home health eligibility requirements
• Strong organizational and multitasking skills
• Excellent communication and phone etiquette
• Proficiency with EMR systems
• Ability to work in a fast-paced environment and meet deadlines
• High attention to detail and problem-solving skills

Preferred Qualifications

• Prior experience in home health or hospice intake
• Familiarity with insurance verification and authorization processes
• Bilingual (English/Spanish) is a plus
