Job description
- The PHN II will provide outreach to the community by means of presentations, displays and handouts on the importance of early and continuous prenatal care as well as health department services available.
- This position will attend and participate in the agency staff conferences, workshops, and in-services as deemed advantageous by the PHN and her or his supervisor. The PHN II will function on agency committees and participate in program audits and evaluations.
- The PHN II will function in the maternal health clinic working directly with clients providing history taking, assessing client needs, and making appropriate referrals to available community resources. Providing maternal health education, HIV counseling, smoking cessation, and other educational needs as deemed appropriate for the client. Appropriate follow-up is ensured for abnormal labs and missed appointments.
- The PHN II will provide pregnancy test counseling and screen clients for presumptive eligibility for Medicaid and refer to community resources as applicable.

- Participate in agency staff conferences, workshops and in-services as deemed advantageous by the SWII and her supervisor. This position will function on agency committees and participate in program audits and evaluations.
- Must work with considerable independence in the clinical or home setting. Work will be reviewed in conferences with the Women’s Health Lead Nurse and by case review.
- Complete timely documentation of visits, contacts, normally within 72 hours of contact. Adhere to agency policies and procedures
- The PHN II will maintain records and reports of services suitable for billing and data collection, adhering to program guidelines. The PHN II will have a working knowledge of local and state program policies and procedures. The PHN II will be expected to work with considerable independence in the clinical setting, with work being reviewed by the nursing director.
- Graduation from a four year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and one year of Public Health nursing experience; or a Master’s in Public Health and graduation from a school of professional nursing and one year of professional nursing experience; or graduation from a school of professional nursing and two years of professional nursing experience, one of which must have been in Public Health; or an equivalent combination of training and experience.
- A current license to practice as a Registered Nurse in North Carolina by the NC Board of Nursing.
- A current North Carolina driver’s license is required.
