Job Summary and Qualifications

Social Worker, Outpatient 

Full time (Monday - Friday, days), no nights, no on call, no weekends, no holidays 

Seeking a Social Worker, Outpatient for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. What you will do in this role: 

• Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support  
• Research and advocate for appropriate public assistance resources for clients  
• Referral to community resources, assistance with socio-economic problems  
• Communicate with client’s care team  
• Provide crisis intervention (referral to DCF, provider, etc as needed)  
• Coordinate treatment planning with specialists and outpatient providers as needed Evaluate current patients for appropriate social work services  
• Judge relative costs/benefits of potential social services and health care actions and choose the most appropriate one. 

What Qualifications you will need: 

EDUCATION:  

Bachelors in Social Work (required)  

EXPERIENCE:  

Experience working with clients that are affected by issues including neglect, abuse, domestic violence, substance abuse • Minimum of 2 years prior experience  

CERTIFICATION/LICENSE:  

State licensure or certification commensurate with degree 

Licensed Independent Clinical Social Worker certification preferred
