Job description

Position Summary
The Clinical Pharmacist provides comprehensive clinical oversight, patient counseling, and operational support within the pharmacy and partnered clinic environments. This role ensures safe, effective, and appropriate medication therapy through clinical review, patient engagement, prescriber collaboration, and quality assurance activities. The Clinical Pharmacist also serves as a key liaison between clinics and pharmacy leadership, supporting workflow efficiency, regulatory compliance, and high‑quality patient care.
Essential Functions amp; Responsibilities
Clinical Oversight amp; Patient Care
  • Provide timely, accurate, and comprehensive drug information—including dosing, compatibilities, and formulary alternatives—to nurses, physicians, and other healthcare professionals.
  • Counsel patients on medication therapy, adherence, side effects, and disease management; maintain accurate and up‑to‑date patient profiles.
  • Review and interpret prescriber orders for appropriateness, ensuring no overdoses, underdoses, interactions, incompatibilities, or unsafe combinations.
  • Actively participate in patient care activities and clinical decision‑making.
Clinic amp; Provider Liaison Responsibilities
  • Serve as the primary point of contact for day‑to‑day clinic issues and questions related to assigned clinics.
  • Act as liaison between clinics and the Clinical Pharmacy Supervisor, communicating changes, requests, and operational needs.
  • Perform routine site visits to assigned clinics, engaging directly with clinic staff and patients to support continuity of care.
  • Consult with prescribers for corrections, clarifications, and therapy optimization.
Operational amp; Workflow Management
  • Monitor daily activities of Specialty Care Representatives, Specialty Administrative Assistants, and Processing Technicians, including workflow, call monitoring, new prescription processing, and hold resolutions to ensure timely drug therapy and proper caseload management.
  • Resolve escalated patient and clinic issues, coordinating with other departments as needed.
  • Quality‑check all prescriptions on hold for future processing.
  • Utilize pharmacy systems to process medication orders and maintain accurate patient information in compliance with HIPAA regulations.
Documentation amp; Reporting
  • Document and report patient adherence and disease‑management activities accurately and in accordance with clinic or contracted MCO requirements.
  • Maintain thorough, timely, and compliant clinical and operational documentation.
Education amp; Experience Requirements
Required
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree.
  • At least one (1) year minimum of retail pharmacy experience.
  • Currently licensed (unrestricted) as a registered pharmacist in the state in which the facility is and any other state as may be required by the pharmacy.
Preferred
  • Specialty pharmacy experience.
  • Previous experience supporting patients living with HIV through counseling, adherence monitoring, and coordination of HIV‑related medication therapy.

Continuing Education Responsibilities/Requirements
  • Maintains all required professional licensure in active and good standing.
  • Maintains adequate and up-to-date knowledge of all common medications and usage.
Physical Requirements / Working Conditions / Behavioral Expectations

  • Ability to walk, stand, sit, and reach frequently throughout the workday.
  • Approximately 10% of travel required (in state and out‑of‑state).
