Care Manager
Location: Nacogdoches Mental Health Clinic - Nacogdoches, TX
Position Number: 1498
Compensation: $42,120 - $43,555 annually, depending on qualifications and experience. Travel reimbursement is provided.
About the Role
Burke is seeking a compassionate and organized Care Manager to join our Integrated Care Clinic team. This position provides care management, care coordination, skills training, and support services to adults, children, and adolescents receiving integrated behavioral and primary healthcare services. The Care Manager works closely with consumers, medical providers, and community partners to improve health outcomes, promote recovery, and ensure continuity of care.
This position primarily serves individuals in Nacogdoches County, with the potential to provide services throughout Burke's service area as needed. The position reports to the Project Manager.
Please note: This position is grant-funded through September 30, 2028, and is contingent upon continued funding.
What You'll Do
  • Provide case management, care coordination, skills training, and other services in accordance with each individual's treatment plan.
  • Coordinate referrals to medical, behavioral health, and community resources.
  • Track referrals, appointments, transitions of care, and treatment outcomes.
  • Follow up on missed appointments and assist consumers in overcoming barriers to care.
  • Collaborate with medical providers, insurance representatives, and community partners to support integrated healthcare services.
  • Maintain accurate, timely, and compliant documentation of all services provided.
  • Travel throughout Burke's service area as needed to provide services and support.

Qualifications
Required
  • Bachelor's degree from an accredited college or university in psychology, social work, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant studies, gerontology, special education, educational psychology, early childhood intervention, or another human services discipline that meets QMHP requirements.
  • Valid Texas driver's license.
  • Ability to complete Satori Alternative to Managing Aggressive Behavior (SAMA), CPR, and First Aid training.
  • Driving record that meets Burke's insurance requirements.
  • Proficiency with Microsoft Office applications.
  • Knowledge of community resources and behavioral healthcare services.
  • Reliable transportation for travel throughout Burke's service area.

Preferred
  • Six (6) months of direct care experience working with adults, children, or adolescents in a behavioral healthcare or primary healthcare setting.
  • Familiarity with DSM-IV or DSM-5 diagnostic criteria.

Work Schedule
  • Monday - Friday
  • Generally, 8:00 a.m. - 5:00 p.m.
  • Regular travel throughout Burke's 11-county service area is required.

Physical Requirements
  • Ability to communicate effectively using visual, auditory, and verbal skills.
  • Ability to attend and lead meetings and training sessions.
  • Ability to drive a vehicle for work-related travel.
  • Ability to lift and carry up to 25 pounds.
  • Manual dexterity is sufficient to operate office equipment and computer systems.
