Work Environment

This role is entirely onsite, allowing for meaningful interactions with clients in their homes. You will collaborate closely with a multidisciplinary healthcare team, including nurses, occupational therapists, and physical therapy assistants, to create an integrative care model. Our home healthcare setting is designed to foster a relaxing and comfortable atmosphere for both patients and staff, ensuring that therapy can take place in a familiar, supportive environment.

Key Responsibilities
  • Conduct comprehensive assessments of clients’ physical abilities and functional limitations using industry-standard diagnostic tests.
  • Design, implement, and adjust individualized treatment plans in collaboration with the clinical team to optimize patient outcomes.
  • Deliver hands-on physical therapy services, including manual therapeutic techniques, exercise instruction, and functional skill training.
  • Oversee and guide Physical Therapy Assistants in their treatment delivery, ensuring adherence to the established care plan and regulatory standards.
  • Evaluate and recommend modifications to clients' home environments to facilitate rehabilitation and independence.
  • Maintain accurate clinical records, including evaluations, progress notes, and discharge summaries, in compliance with agency policies.
  • Provide education to clients and families regarding physical therapy programs and the use of therapeutic devices.
  • Participate actively in multidisciplinary team meetings, providing insights into clients' progress and suggesting coordinated care strategies.
Patient Population / Client Focus

This position primarily involves working with a diverse range of clients, including individuals recovering from surgeries, those with chronic pain conditions, and patients in need of rehabilitation support following major health episodes. Your work will greatly impact clients’ abilities to perform daily tasks independently, enhancing their quality of life and promoting overall well-being.

Qualifications
  • A Doctorate degree in Physical Therapy (DPT) is required.
  • Current, valid license to practice as a Physical Therapist in the state of Virginia.
  • At least two years of experience in a physical therapy role, preferably in an acute care or home care setting.
  • Current CPR certification to ensure patient safety.
  • Possession of a valid driver’s license and proof of automobile insurance.
  • Demonstrated commitment to clinical excellence and patient-centered care.
  • Strong interpersonal skills to foster effective communication with clients, families, and team members.
  • Capability to adapt to new challenges and provide innovative solutions in patient care.
Preferred Experience
  • Experience in home health care environments is highly desirable and will be beneficial in providing tailored therapy solutions.
  • Knowledge of current trends and best practices in physical therapy, including adaptations for various disabilities.
  • Experience with electronic health records (EHR) for maintaining client documentation and reporting.
Compensation and Benefits

This full-time position offers an annual salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications. In addition to competitive pay, we provide a comprehensive benefits package that includes:
