Berkley Home Health Care is seeking highly skilled and compassionate Registered Nurses (RNs) to join our team on a per-visit basis in the North Denver territory of (Thornton, Arvada. Westminster, Broomfield, Northglenn), The RN is responsible for providing comprehensive, skilled nursing services to patients in their homes, following physician-ordered plans of care and adhering strictly to all federal, state, and CMS/CHAP regulatory requirements.

What You’ll Do

• Perform admissions, recerts, and routine skilled nursing visits (OASIS)
• Deliver high-quality care: wound care, med management, IVs, disease education (CHF, COPD, Diabetes)
• Educate patients and families to prevent rehospitalizations
• Collaborate with physicians and our clinical team to adjust care plans as needed
• Complete timely, compliant documentation in our EMR

What You Need

• Active Colorado RN license
• 1+ year RN experience (home health preferred, but we’ll train the right candidate)
• Strong clinical skills (wound care, IVs, assessments)
• Reliable transportation
