Key Responsibilities

• Develop and implement personalized treatment plans in collaboration with medical teams to address the varied psychosocial concerns of patients.
• Coordinate with community organizations to arrange post-discharge services, ensuring ongoing support and continuity of care for patients.

Qualifications
