Job Title:        Social Services Manager – AgeGuide Caregiver Resources Grant

Position Description: Provide training and education through outreach and care coordination for caregivers per AgeGuide requirements. 

1. Approximately 85% of your work hours will involve traveling to communities across Lake County to engage them in support of AgeGuide initiatives.
2. Facilitate existing caregiver group
3. Provide T-Care assessments
4. Start new caregiver groups and Memory Café’s
5. Assist with data entry for the AgeGuide.Title IIIE.Caregivers Resources
6. Assist with reporting.
7. Assist with vendor invoices.
8. Provide Information and referral.
9. Assist with outreach per AgeGuide requirements.
10. Attend workshops and professional education sessions as required by AgeGuide.

Minimum qualifications sought:

1. BS, Social Work degree preferred. Experience working with older adults and/or their caregivers preferred.
2. Must have good verbal and written skills.
3. Must have good computer skills.
4. Must have dependable transportation
5. Must have background and driver’s license checks prior to employment

Hours:    Fulltime/Parttime position. Some flexibility in hours worked.
