Position Overview

We are seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) for a full-time position within our community health clinic located in Robinson, Pennsylvania. Work Environment

This position offers you the flexibility of a remote work setting, ensuring you can maintain work-life balance while effectively reaching and supporting clients. You will be part of a closely-knit team focused on providing exceptional mental health services. Our typical work schedule runs Monday to Friday, from 9:00 AM to 5:00 PM, allowing for structured interaction with both clients and colleagues.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to understand patients' needs, strengths, and challenges, and develop effective intervention strategies.
  • Provide high-quality counseling, health education, and disease management services, tailored to the unique circumstances of each client.
  • Create individualized care plans in collaboration with patients and an interdisciplinary health care team, focusing on behavior modification and lifestyle changes that support mental health.
  • Encourage goal setting for patients both synchronously during sessions and asynchronously through follow-up communications to foster long-term engagement and success in managing chronic conditions and mental health needs.
  • Carry out regular outreach to patients, ensuring adequate support and guidance based on their expressed needs and preferences to facilitate positive behavior change.
  • Document all patient interactions and care plans promptly and accurately in our electronic health record (EHR) system, maintaining compliance with all regulatory standards.
Patient Population / Client Focus

You will engage with a diverse patient population, encompassing various backgrounds, identities, and challenges. This role emphasizes cultural competence, as you will be working with individuals facing a range of mental health issues, chronic conditions, and psychosocial stressors, requiring tailored approaches and empathetic understanding.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • An active state license as a Licensed Clinical Social Worker (LCSW) must be held in good standing.
  • A minimum of three years of practiced experience in mental health or outpatient settings, demonstrating a strong foundation in clinical practices.
  • Comprehensive knowledge of evidence-based therapeutic modalities and treatment approaches is essential.
  • Superior interpersonal, communication, and assessment skills to effectively engage with clients and collaborate with interdisciplinary teams.
Preferred Experience

Experience with specific evidence-based practices such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), or trauma-informed care is highly desirable. Familiarity with working in a community health setting will enhance your capability to tackle diverse challenges faced by clients.

Compensation and Benefits
