PURPOSE OF THE POSITION

This supervisory position oversees Supports Planners (Case Managers) and is ideal for professionals seeking to advance their careers in care coordination within Maryland's home and community-based services programs.
As a Team Manager, you will lead a team of Support Planners, ensuring high-quality, person-centered service delivery. You will also carry a limited caseload. The primary goal is to provide remote training, resources, and oversight to your team.

Essential Responsibilities
  • Supervise, mentor, and evaluate a team of Supports Planners/Case Managers, including assigning caseloads, monitoring performance, providing guidance, and ensuring compliance with organizational policies, contractual billing guidelines, and Maryland Medicaid regulations
  • Oversee team activities related to client needs assessment, development and monitoring of person-centered Plans of Service, telephonic and on-site care coordination, crisis resolution, and resource linkage
  • Carry a select caseload: evaluate clients� capacities, strengths, and support networks; gather information; develop and monitor Plans of Service; conduct telephonic contacts and on-site visits; promote positive health behaviors and self-direction; assist with transitions to community independence; and collaborate with Social Workers, Registered Nurses, community agencies, and providers
  • Facilitate team collaboration to address complex medical/psychological needs and achieve optimal client outcomes
  • Implement critical thinking to resolve client crises and manage multiple priorities with flexibility
  • Ensure accurate documentation, real-time data entry, and adherence to billing and reporting standards
  • Educate clients on self-direction philosophy and access to resources
  • Adapt to policy/procedural changes and uphold the organization's core values, mission, and commitment to those served

Qualifications
  • Bachelor's degree from a four-year college or university in health/human services or related field (e.g., psychology, sociology, counseling, social work, nursing, physical therapy, or equivalent)
  • Minimum 3�5 years of experience in case management, supports planning, or related field, preferably with supervisory or lead responsibilities
  • Strong oral and written communication skills; ability to communicate effectively with individuals of varied cognitive abilities and build professional relationships
  • Proficiency in Microsoft Office Suite (Word, Excel, Access, Outlook, PowerPoint) and ability to conduct computer-based research
  • Ability to read, interpret, and apply documents such as assessment reports, procedure manuals, and employment paperwork
  • Valid Maryland driver's license, reliable transportation, and willingness to conduct on-site client visits
  • Proven ability to work independently, manage schedules, prioritize tasks, and handle multiple responsibilities with minimal supervision
