Avidity Integrated Care

We are an agency that provides varying services for individuals with intellectual and developmental disabilities. We are seeking new members to join our team in caring for the patients we serve. The following is expected of each potential candidate:

Role Responsibilities

• Establishes a caring therapeutic relationship with clients. Provides appropriate habilitative and personal care while ensuring that clients’ safety, health and well-being are maintained.
• Correctly implements Individual Support Plan/Plan of Care including participating with the client’s during activities in the community as defined in the Individual Support Plan.
• Demonstrates an understanding of client rights and confidentiality and ensures these rights are protected.
• Demonstrates an understanding of communication techniques/strategies to support the client and the person-centered approach.
• Participates in initial and ongoing Individual Support Plan development.
• Provides transportation for clients as defined in the Individual Support Plan/Plan of Care.
• Provides appropriate level of supervision as outlined in the Individual Support Plan.
• Demonstrates knowledge of Avidity Integrated Care Policies and Procedures defined in the employee handbook for direct care staff.
• Completes documentation as required. Records accurate, informative, and appropriate notes which reflect applicable goals.

The following items are needed to move forward in the hiring process:

- Driver's License
- Social Security Card
- High School Diploma or GED
- First Aid and CPR certification (on-site training available)
- Proof of current car insurance/registration
