Key Responsibilities

As an Outpatient Physical Therapist at Aviant Physical Therapy, you will:

  • Conduct comprehensive evaluations by analyzing relevant medical history, performing targeted assessments, and documenting results effectively.
  • Communicate evaluation findings and treatment plans clearly to patients and their families, involving them in the goal-setting process.
  • Implement tailored treatment protocols grounded in evaluation results and adjusted as necessary based on patient progress.
  • Work in conjunction with a multidisciplinary team, consulting with other healthcare professionals to modify treatment plans in response to observed changes in patients' conditions.
  • Prepare and implement discharge plans and ensure appropriate follow-up care while maintaining accurate patient records, including evaluations, encounter notes, progress summaries, and discharge evaluations.
  • Manage your workload effectively to meet productivity expectations, maintain an organized schedule, and contribute to community outreach initiatives.
  • Engage in departmental goals and objectives, ensuring compliance with relevant regulations and standards.
  • Initiate your professional development through continuing education programs and mentorship opportunities for students in the field.
  • Foster an exceptional patient experience by being responsive to their needs and promoting access to care.
Patient Population / Client Focus

Qualifications

The ideal candidate for this role will possess:

  • A Doctorate of Physical Therapy (DPT) degree.
  • A valid physical therapy license in the state of New Jersey.
  • Current American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers.
