Our community health clinic specializes in a multidisciplinary approach that combines compassion and evidence-based therapies to promote improved mental health outcomes. Our team consists of well-trained professionals who are committed to supporting clients through their mental health journeys.

Work Environment

The work setting is remote, allowing for a flexible work-life balance while maintaining a dedicated focus on patient-centered care. You will be part of a close-knit team, collaborating with other social workers, psychiatrists, and healthcare professionals, using telehealth technology to reach and assist our patients effectively. Regular team meetings and professional development opportunities are integral parts of our remote work culture.

Key Responsibilities

As a Licensed Clinical Social Worker at Atlantic Health P.C, your responsibilities will include:

  • Conducting comprehensive psychosocial assessments to evaluate clients’ mental health status and identify strengths and needs.
  • Implementing evidence-based therapeutic interventions, including cognitive-behavioral and dialectical behavior therapy, tailored to each patient's unique situation.
  • Working collaboratively with a multidisciplinary team, including psychiatrists and nurses, to create a coordinated care plan that addresses physical, emotional, and psychological needs.
  • Providing education and support to patients and their families regarding diagnoses, treatment options, and medication management to empower informed decision-making.
  • Facilitating individual, group, and family therapy sessions employing effective evidence-based modalities that promote healing and personal growth.
  • Developing personalized treatment plans in partnership with the client and treatment team, ensuring that each client’s goals and preferences are central to their care.
  • Assisting clients in navigating the often complex mental health system and advocating for their rights, ensuring they receive the services and support they need.
  • Managing discharge planning, therapeutic group sessions, and psychoeducational activities to provide ongoing support and resource connection.
Patient Population / Client Focus

You will work with a varied population of clients, including children, adolescents, and adults who present with a range of mental health disorders such as anxiety, depression, PTSD, and other challenges impacting their well-being. Our commitment is to provide inclusive care that respects each individual's unique circumstances and cultural backgrounds.

Qualifications

We require candidates to meet the following qualifications:

  • A Master’s degree in Social Work from an accredited program.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW).
  • A comprehensive understanding of mental health disorders, treatment modalities, and effective crisis intervention strategies.
  • Strong capability to collaborate within a multidisciplinary team to enhance patient care.
Preferred Experience

While experience in clinical social work is preferred, we also value previous work or internship experiences that demonstrate a commitment to mental health advocacy and patient care, particularly within community health environments.
