Key Responsibilities

• Conduct comprehensive psychosocial assessments by interviewing patients, observing their interactions, and reviewing pertinent medical records to identify emotional, social, and environmental factors that may affect their recovery process.
• Provide compassionate counseling and support to patients and their families, effectively addressing issues related to emotional adjustment and overall mental well-being.
• Collaborate closely with our interdisciplinary rehabilitation team to develop and implement individualized treatment plans that account for each patient’s unique psychosocial needs.
• Assist patients in navigating and accessing essential community resources, such as support groups, vocational rehabilitation services, and housing assistance, to enhance their recovery journey.
• Educate patients and families about available resources and post-rehabilitation services, ensuring they understand how to utilize them effectively upon discharge.
• Participate actively in discharge planning, facilitating seamless transitions for patients returning home or moving to other care settings.

Patient Population / Client Focus

Our patient population includes individuals recovering from a variety of conditions such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Our LCSW will work diligently to address the unique emotional and social challenges each patient faces, guiding them toward reintegration into their communities and fostering their journeys toward full recovery and independence.

Qualifications

• A Master’s degree in Social Work from an accredited program is required.
• Must hold an active state license as a Licensed Clinical Social Worker (LCSW).

Preferred Experience
