JOB TITLE: Home Delivered Meal Program Coordinator

REPORTS TO: HDM Director

SUPERVISES: none

FLSA CLASSIFICATION: hourly

STARTING SALARY RANGE: $19.80-$24.20

JOB SUMMARY: The Home Delivered Meal Coordinator is responsible for overseeing the route management, client care, and customer service functions of the Home Delivered Meal Program. This position works closely with Home Delivered Meal Warehouse Coordinator and reports to the HDM Director.

ESSENTIAL JOB FUNCTIONS:

· Manage all Aging Best home delivered meal routes, including adding new clients into routes, creating route instructions, making updates to personal information, and changing client status as needed in the client management system.

· Complete initial assessments and/or annual nutrition risk assessments in a timely and professional manner for all Aging Best program HDM clients.

· Enter data as required to ensure that all services are entered and all-important information is documented appropriately in the client management system.

· Respond to all concerns and inquiries from current HDM clients, family members, or the public about the program, or particular situations as they arise in a timely, supportive, and professional manner.

· Work with the Home Delivered Meal Warehouse Coordinator to assist in the day-to-day support and oversight of all Meals on Wheels and HDM program volunteers.

· Complete a stable shelf meal and other production operations in the warehouse as available.

· Assess opportunities to improve efficiency and streamline processes

· Encourage and reinforce innovation

· Recruit new participants/clients/volunteers and oversee expansion and growth of programs

· Promote awareness of Aging Best and the program through participation in community events and networking with other community, regional and state organizations to promote a culture of health and wellness

· Complete all necessary daily, weekly and monthly reporting requirements in a professional and timely manner.

· Think strategically and present solutions and innovations

PRE-REQUISITES:

Education: A High School Diploma or GED.

Experience: Four years practical experience in one or more of the following areas: food service, warehouse, fleet management, shipping/receiving, social service, or community relations.

Physical: Reliable and predictable attendance is expected and needed for this job. Moderate physical activity is required. Ability to push, pull, or lift up to 50lbs with or without reasonable accommodation. Requires ability to be mobile and operate a motor vehicle including a box truck. Subject to repetitive motions such as typing. Requires extensive communication skills and the ability to be understood by others.

Travel: Requires moderate amounts of travel within the region and occasional travel outside the region. Must have current motor vehicle operator’s license, proof of automobile insurance and reliable transportation.

Legal: Aging Best conducts criminal background records check, a motor vehicle driving record check, sexual predator’s list check, and Missouri Employee Disqualification List check as a condition of employment.
