Key Responsibilities
  • Conduct comprehensive evaluations and assessments to identify the developmental and emotional needs of children under three years old.
  • Collaborate with families and multidisciplinary teams to develop and implement Individualized Family Service Plans (IFSPs), ensuring tailored interventions are in place.
  • Track and monitor client progress, adjusting treatment approaches as necessary to meet evolving needs and promote optimal outcomes.
  • Provide guidance and support to families, ensuring they are equipped with strategies and resources to aid their child’s development.
  • Maintain compliance with all relevant federal and state regulations while effectively documenting assessments, plans, and outcomes.
  • Administer various rating scales and developmental assessments to evaluate client needs accurately.
  • Develop rapport with children, caregivers, and team members to promote a supportive and trusting environment conducive to learning and growth.
Patient Population / Client Focus

In this role, you will focus on a vulnerable population—children with disabilities or developmental delays aged 0-3 years. Your work will directly impact their ability to acquire essential skills in areas such as physical, cognitive, emotional, and communication. Supporting families during this crucial developmental phase is at the heart of our mission, as we strive to empower both children and their caregivers.

Qualifications
  • A Master’s Degree in Social Work (MSW) or Doctorate of Social Work (DSW) from an accredited university.
  • Current Licensure as a Clinical Social Worker (LCSW) by the Department of Public Health.
  • A valid driver's license to facilitate travel to client homes or community-based locations as required.
  • Adaptability in employing various methods to cater to diverse learning styles of children and their unique treatment plans.
  • Strong commitment to Natural Learning Environment Practices.
Preferred Experience
  • At least 2 years of professional experience working in early intervention settings with children and families.
  • Proficiency in using assessment tools and developing intervention strategies aligned with evidence-based practices.
  • Experience collaborating with multidisciplinary teams to deliver comprehensive care.
Compensation and Benefits
