Position Overview

Advanced Cardiology PC is seeking a dedicated and compassionate Nurse Practitioner to join our team in Oswego, New York. Work Environment

The work environment at Advanced Cardiology PC is collaborative and supportive, designed to empower healthcare providers. As a Nurse Practitioner, you will work within a vibrant outpatient clinic team that operates from Monday to Friday, 9:00 AM to 5:00 PM. The focus here is not only on treating illness but also on understanding patients’ needs holistically, enabling comprehensive care while reducing hospitalizations. Our facility is equipped with the necessary tools and technology to facilitate effective communication and patient management.

Key Responsibilities
  • Deliver exemplary primary care services, ensuring all patient visits are thorough and compassionate.
  • Conduct office visits for both routine check-ups and acute health issues, using your assessment skills to evaluate and treat patients appropriately.
  • Perform annual wellness visits and health risk assessments with a focus on accurate documentation and patient education regarding their health.
  • Coordinate care with other healthcare providers, specialists, testing facilities, and agencies, ensuring seamless patient transitions and continuity of care.
  • Assume a leadership role in population health initiatives by ensuring screenings and preventive measures are articulated and completed in collaboration with the care team.
  • Assist with phone triage and patient outreach, ensuring that patients receive timely responses to their queries and concerns.
  • Educate patients on their diagnoses, treatment plans, and management strategies to empower them in their healthcare journeys.
  • Participate in community promotional activities to raise awareness of our services and the importance of preventive health.
  • Conduct home visits as deemed necessary, catering to patients who may have difficulty accessing our facility.
  • Report to the Medical Director and collaborate closely with operational leadership to ensure alignment with organizational goals.
Patient Population / Client Focus

The core focus of this role is to provide personalized primary care for older adults on Medicare. We understand the unique challenges faced by this demographic and aim to address their needs with empathy and expertise. Our care model is community-centered, resulting in tailored healthcare experiences that prioritize the quality of care over the quantity.

Qualifications
  • Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
  • National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
  • Possession of an active, non-probationary state Nurse Practitioner license.
  • In possession of an active DEA license.
  • Experience working in primary care, particularly in internal medicine, geriatrics, or family medicine.
  • Proficient in HCC documentation, ICD-10 coding, and health risk assessments.
  • A passion for teamwork and a collaborative spirit are essential to successfully integrate into our cooperative care team.
Preferred Experience
  • Experience with electronic health records (EHR) systems that enhance patient documentation and care coordination.
  • Familiarity with population health management strategies and community outreach initiatives.
Compensation and Benefits

The salary for this position ranges from USD 120,000 to USD 160,000 annually, commensurate with experience and qualifications.
