Key Responsibilities

As a Physical Therapist at Ability Capacity Beyond Inc., your day-to-day activities will include a variety of responsibilities aimed at helping patients achieve their rehabilitation goals:

• Conduct comprehensive Physical Therapy evaluations for children and adults to assess their baseline functional abilities and specific needs.
• Develop, implement, and modify individualized treatment plans based on evaluations and ongoing assessments.
• Utilize evidence-based practices to track patient progress and adjust treatment protocols as necessary.
• Prepare and maintain accurate documentation such as SOAP notes, progress reports, and care plans utilizing our electronic health record system.
• Educate patients and their families about treatment processes, expected outcomes, and potential challenges during recovery.
• Meet performance benchmarks and participate in regular team meetings to discuss patient progress and treatment strategies.
• Collaborate with interdisciplinary teams within Ability Capacity Beyond Inc., ensuring an integrated approach to patient care.
• Promote the company's mission and values, serving as an ambassador for our dedication to improving patient outcomes.

Patient Population / Client Focus

You will work with a diverse patient population, ranging from children to adults facing various physical challenges. This role allows you to treat a wide array of conditions, enabling you to make a profound difference in their recovery journeys. Qualifications

The successful candidate for this Physical Therapist position will possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited program; a Master's or Doctorate in Physical Therapy (DPT) is preferred.
• Active and unrestricted licensure to practice in the state of Connecticut.
• Familiarity with both in-person and telehealth therapy modalities, adapting to the clinical requirements of the situation.
• A genuine commitment to working with patients from varied backgrounds, embracing flexibility and teamwork in a dynamic healthcare environment.
• Strong communication skills and a collaborative mindset, allowing for effective teamwork and positive patient interactions.

Preferred Experience

Experience in outpatient physical therapy settings, particularly involving pediatric care, is advantageous. Familiarity with electronic health records and a proactive approach to professional development is also preferred, as we encourage continuous learning and improvement.

Compensation and Benefits
