Description:
Our Home Health Occupational Therapists (OT) are the heart of our organization! In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience. Responsibilities:
  • Evaluate patient's functional status and home environment
  • Identify equipment needs and participate in the development of the total plan of care
  • Develop treatment program and establish goals for improved function
  • Teach new skills, retrain in once familiar daily activity skills, fabricate splints, train in use of prosthetic and/or orthotic devices
  • Maintain appropriate clinical records and communication with other personnel involved in patient's care
  • Supervise Occupational Therapy Assistants according to organization policy and state regulations
  • Participate in discharge planning
  • Participate in case conference as required
  • Ensure compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned

Qualifications:
  • Graduate of an occupational therapist education an accredited program with current licensure to practice in state of operation
  • 2+ years occupational therapy experience
  • Home health experience preferred.
  • Commitment to clinical and documentation excellence
