Case Manager - DDD Home Care


Location: Jackson, NJ (on-site)
Job Type: Full-time
Salary: $60,000–$70,000 per year, based on experience.


About ASG Home Care

ASG Home Care is a growing New Jersey DDD-licensed agency supporting adults with developmental disabilities. We are a small, hands-on team where good work gets noticed and problems get solved quickly. We are hiring a Case Manager to own the care side of our operation.


How This Role Works

You will not be buried in administrative work. You'll be paired with a dedicated remote administrative coordinator who works hand-in-hand with you daily — they own DSP-focused administration, scheduling mechanics, and compliance documentation. You own the human side and the decisions:


What You'll Own

  • Case management — a caseload of individuals receiving services, their plans, goals, and service delivery from intake through ongoing care

  • Support Coordinator relationships — being the reliable, responsive point of contact SCs trust with their referrals

  • Families — communication, problem-solving, and confidence-building with the people who count on us

  • Staffing decisions — matching the right DSP to each individual and case, evaluating fit, and making coverage calls (your coordinator handles the scheduling legwork; you make the judgment calls)

  • Case documentation quality — ensuring plans and case records meet DDD standards, with compliance administration supported by your coordinator


What We're Looking For

  • Experience in the DDD/IDD world — Case Manager, Support Coordinator, DSP supervisor, or program coordinator background strongly preferred

  • Someone who fixes problems without being asked twice

  • Organized and reliable — deadlines and documentation never slip on your watch

  • Strong communicator by phone, text, and in person — with families, SCs, and staff

  • Comfortable in a small, fast-moving company where you'll wear more than one hat

  • Valid driver's license and reliable transportation

  • Bilingual English/Spanish a plus


What We Offer

  • $60,000–$70,000 salary based on experience
