About the Role:
Join ALIGN HOSPICE CARE LLC as a compassionate LPN in Snellville, GA, where you'll make a meaningful impact in the lives of our patients and their families. We are dedicated to providing exceptional hospice care that emphasizes dignity, comfort, and quality of life.
Responsibilities:
- Provide direct patient care, including administering medications and monitoring vital signs.
- Assist with daily living activities and personal care for hospice patients.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Educate patients and families on care options and end-of-life issues.
- Document patient progress and communicate changes to the nursing staff and physicians.
- Participate in team meetings and contribute to quality improvement initiatives.
- Ensure compliance with all regulatory and safety standards in hospice care.
- Provide emotional support to patients and families during challenging times.
Requirements:
- Current LPN license in the state of Georgia.
- Minimum of 1 year experience in hospice or palliative care preferred.
- Strong clinical assessment and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate demeanor with a commitment to patient-centered care.
- Basic Life Support (BLS) certification required.
- Flexibility to work various shifts, including weekends and holidays.
- Reliable transportation and willingness to travel to patient locations.
