An Occupational Therapist is needed for a full‑time day‑shift contract at a skilled nursing facility in Brooklyn Park, MD (Hammonds Lane area), serving adult and geriatric residents in post‑acute rehab and long‑term care. This 14‑week new opening runs standard weekday/day hours with a 35‑hour weekly guarantee and is focused on functional recovery, ADLs, and safe discharge.

Setting: Skilled Nursing Facility (SNF) / rehab center
Position type: Contract, full-time, non‑permanent
Contract length: 14 weeks (02/23/26 – 05/25/26)
Schedule: 8:00 AM – 4:30 PM, 30‑minute unpaid meal break (5x8 days)
Days: Facility‑set day schedule (Mon–Fri typical; weekend availability may be requested)
Guaranteed hours: 35 hours per week
Blended Gross Rate: $45–$55

Key responsibilities
Evaluate residents’ ability to perform ADLs, functional mobility, and self‑care in a SNF rehab setting.
Develop and implement individualized OT care plans to improve independence, safety, and quality of life.
Provide interventions including ADL retraining, therapeutic exercise, neuromuscular re‑education, balance/coordination training, and adaptive equipment training.
Collaborate with PT, SLP, nursing, and social work on interdisciplinary care plans and discharge goals.
Accurately document evaluations, daily treatment notes, progress notes, and discharges in the EMR.
Educate residents and caregivers on home programs, energy‑conservation techniques, and safe use of equipment.
Required qualifications and skills
Active Maryland Occupational Therapist license (or eligibility before start).
Graduate of an accredited OT program.
Recent SNF or geri‑rehab experience strongly preferred.
Current BLS/CPR for healthcare providers.
Strong documentation, time‑management, and interdisciplinary communication skills.
