Greater Nashua Mental Health is seeking a compassionate and motivated Case Manager to join our Assertive Community Treatment (ACT) team. In this role, you'll work directly with individuals experiencing severe and persistent mental illness, helping them access services, build independence, and achieve recovery goals in their own communities.ย

Key Responsibilities

  • Provide case management and direct support services to individuals with complex mental health needsย 
  • Work within a multidisciplinary ACT team to coordinate treatment plans and servicesย 
  • Support clients with medication monitoring, crisis intervention, and daily living skillsย 
  • Advocate for clients and connect them with community resourcesย 
  • Deliver services in community and natural environments to support recovery and independence
  • Assist with daily medication runs on a rotating schedule
  • Participate in a rotating on-call schedule 1x per week

Qualifications
  • Bachelor's degree in Social Work, Psychology, Human Services, or related fieldย 
  • 1-2 years of related experience preferredย 
  • Strong assessment, diagnostic, crisis intervention, and de-escalation skills.
  • Strong communication, collaboration, and problem-solving skillsย 
  • Ability to work in a fast-paced, team-based environment
