Job Title: Child-Youth Case Manager
Location: Arizona
Employment Type: Full-time
Salary: $19 - $21/hr.

About Your Role
Our Child-Youth Case Manager works with children and youth in the community, engaging them in the creation of their own service plan and goals, providing social skills and behavior management support/education, and advocating/linking them to additional community supports. You will meet with your clients and families in their homes, in the community, and in the office, to support their progress and assess needs to adjust the treatment plan as necessary. You will also have weekly contact with your clients.
 
 
Responsibilities 
●        Work directly with children and youth to help create personalized service plans and goals
●        Provide social skills education and behavior management support
●        Advocate and connect clients to community resources
●        Meet with clients and families in various settings (homes, office, community)
●        Assess progress and adjust treatment plans as necessary
●        Facilitating CFT’s at a minimum monthly 

Required Qualifications

●        Bachelor’s or Master’s Degree in a related field (Psychology, Social Work, etc.)

●        CPR/First Aid Certification

●        Valid AZ Driver’s license without restrictions, current auto insurance, and own transportation

●        Eligible for fingerprint clearance approval throughout employment

Preferred Qualifications

●        Experience in Case Management

●        Experience in Behavioral Health

●        Bilingual: English/Spanish
