About the Opportunity: 

Help patients with the care they need in the comfort of their home. Comprehensive Care Management opportunities to provide home health through team approach for continuity of care in each patient's home setting.

Treating Patients with and who have some self-care/caregiver support:

  • Acute and post-acute care
  • COPD/Asthma
  • Cellulitis
  • Dehydration
  • Gastroenteritis
  • Heart Failure
  • Altered Mental Status
  • Pneumonia
  • COVID
  • Diabetes
  • Urinary Tract Infection

Position Details:

  • This is a W2 position 
  • Flexible schedulework as much or as little as you want, whenever it suits you
  • Opportunities to cover 12- hour shifts
    • Shifts are from 8a-8p and 8p-8a, 7 days per week
  • Provide daily clinical guidance and decision-making as the attending Advanced Care Physician
  • Collaborate with Primary Care Physicians and offices, supported by a multidisciplinary Care Team of Nurses, Nurse Practitioners, Medical Assistants, and Care and Case Managers
  • Patient visits and on call responsibilities; be available for ongoing orders, next steps, coordinating care with staff, and communicating with primary care provider
    • Answering service takes after hours call first, then dispatched out to team
  • NextGen EMR
  • See 10-12 patients per day on average for follow-up days
  • See 5-6 patients per day on average for new patients

Coverage Areas:

  • St. Louis
  • St. Louis County
  • St. Charles
  • Jefferson County

Compensation for 12-hour shifts:

  • Weekday shift rate- $1800.00 
  • Weekend shift rate and holiday rate- $2000.00
  • On-call rate- $150.00 
    • $265.00 per hr, (min of 2 hrs) for in-home visits outside of scheduled shift

Preferred Requirements: 

  • Hospitalist or home health experience 
  • Experience and training in quality improvement and patient safety
  • Able to create personal relationships and build human connections
  • Communicate effectively with patients, family, medical power of attorney, primary care provider and all individuals involved in the patients care
  • Provide care with compassion, empathy, and cultural competency
  • MD or DO
  • Must be Board Certified or Board Eligible with plans to certify (AB or AOA)
  • Active Missouri Medical License or eligible to obtain licensure in Missouri
  • Enrolled in Essence and other Medicare HMO plans
  • ACLS Certified
  • Good driving record and auto insurance
